Moved: Architecture Decision Records

The ADRs now live under ../explanation/adr/, following the handbook docs contract. Individual records keep their numbers, e.g. 0007-agent-trust-model.md is now ../explanation/adr/0007-agent-trust-model.md.

This stub remains only so existing external links to docs/decisions/ do not break.
